Making an extraordinary difference to young people with complex needs

Demelza’s transition team supports over 200 families, guiding young people aged 14–24 through the complex shift from child to adult services. From benefits advice to peer support and social activities, the service fills a vital gap and helps families feel safe, connected, and understood.

“Fenton is 17, so at the moment he is still accessing all of Demelza’s services. I appreciate that numerically, maybe, Fenton is nearly an adult now, but actually nothing has changed. If anything, as he’s got older, his needs have got more profound."

Ivy's transition story

“Ivy is 18 now, but she’s still a very complex little girl and requires 24/7 care. She has a condition called mitochondrial cytopathy, which has left her with complex needs, including profound multiple learning difficulties. Ivy is non-verbal and she is 100% reliant on a wheelchair, which supports her. She can generally express herself through excitement, or anger, or she can just laugh her head off, especially when she hears music!"
